FAA Medical certificate class 1., 2. a 3. for pilote licence issued in USA

I issue Medical Certificate class 1.,2. and 3. according to US FAA rules. Those who have a pilot's license issued in the USA, fly an aircraft registered in the USA, want to obtain a pilot's license in the United States of America or want to fly in a country that recognizes the American medical certificate need such a Medical certificate. After studying in the USA, I was appointed by the Federal Aviation Administration as an International AME.

Before the examination in my office, you should fill out an application and a health questionnaire online, which is available in electronic form at https://medxpress.faa.gov/. First, create an electronic account at this FAA website. You will then get access to the questionnaire, which I recommend you fill in very carefully. After completing the questionnaire, you will receive a numerical code on this website. You must give this code to me in my office during the examination or send it to me by email before the examination in my office.

I try to have sensitive and professional approach to fellow pilots, so that the examination is not stressful or unpleasant. I maintain modern approach to aviation medicine, corresponding with the current state of knowledge in medicine.

What is the procedure of the medical exam?

  • I make an appointment for an examination at a specific hour and make an appointment throughout the day.
  • It is not necessary to come early in the morning, but at the agreed hour. I carefully make sure that the pilots do not wait at my place and I respect the time of the order.
  • The examination in my outpatient clinic takes about 60 minutes, during which I constantly attend to you.
  • I will issue the Medical Certificate immediately after the examination, unless the Part 67 regulation instructs me otherwise.
  • It is not necessary to come to the examination on an empty stomach.
  • According to the regulations, the examination must be conducted in English. The FAA requires English language proficiency to issue a Medical Certificate.

Who can come to me for a medical exam for the FAA Medical Certificate?

  • Anyone can apply for a Medical Certificate regardless of age or pilot license, whether they fly with any motorized aircraft or helicopter. For gliders and pilots with a Sport Pilot license, a Medical Certificate is not required in the United States.
  • Anyone who has a pilot license issued in the USA, flies with an aircraft registered in the USA or wants to fly in a country that accepts the American medical certificate.
  • Anyone who wants to undergo pilot training in the United States.
  • Any holder of an FAA Medical Certificate.
  • The Medical Certificate issued by me must be accepted by all authorities and operators in the United States and other countries where the FAA Medical Certificate is accepted.
  • I also extend limited certificates and certificates issued on the basis of an Special Issuance, if you suffer from a serious illness.
  • I can renew or reissue the Medical Certificate at any time, whose validity has already expired.
  • I am able to perform renewals even in case of serious health problems according to FAA rules.

What medical exams do I perform to renew the FAA Medical Certificate?

  • I only perform examinations that are required by the rules of Part 67 - Medical standards and certification.
  • I do not perform any additional examinations that are not required by the regulation or their need did not arise due to the health condition of the pilot.
  • I do not perform any traditional psychotests.
  • I have all the necessary equipment and instruments for a standard examination. Unless there is a reason arising from your possible illness, I will not send you to any other doctors and you will not wait in any other waiting room.
  • I perform clinical examination, ECG, laboratory tests, eye examination, ENT examination and urine test as specified in Part 67.
  • Only under justified circumstances is it necessary to supplement other examinations.
  • Serious health problems and deviations defined in the FAA AME Guide must be consulted by every Aeromedical Examiner designated in the USA with Federal Flight Surgeons in Oklahoma. Due to the time difference and distance, such a consultation can take several days and delay the issuance of the Medical Certificate.

Where and for when can I make the medical exam appointment?

  • Make your appointment over the Internet, in Prague and Ostrava.
  • It is also possible to make your appointment by phone (+420 603 423 470), e-mail (melechovsky@leteckylekar.cz) or via Facebook.
  • You can make your appointment by phone every working day from 8 to 19 o’clock.

  • If you are permanently taking any medication or are being treated for a chronic disease, please let me know when you make your appointment. In this way, we can agree in advance on any necessary additional examinations and I will save you one trip to my ambulance.

What should I take with me for the medical exam?

  • Before the examination, you must fill out an online application and health status questionnaire, which is available in electronic form at https://medxpress.faa.gov/. Without a completed application, it is not possible to proceed with the inspection according to FAA rules.
  • After completing the questionnaire, you will receive a confirmation number, which you must give to me in my surgery during the examination, you can forward it to me by email.
  • If you have obtained an Authorization for Special Issuance or a Statement of Demonstrated Ability in the past, you must bring these documents to the examination.
  • Take your ID or passport with you. If you have already had an FAA Medical Certificate issued in the past, please bring it to the examination.
  • If you wear glasses, you must bring them to the examination. It is possible to bring contact lenses to the examination, but in certain circumstances I have to examine you without them and it is necessary to remove them from your eyes in my office. If you feel that your glasses do not suit you and you do not see optimally in them, first visit an optician and ask him for new glasses.
  • You do not need to carry a "medical report" with you from the medical facility where you were last examined as a pilot. I don't even require a general practitioner's report or other medical documentation.
  • If you suffer from a more serious illness, please inform me in advance when making your appointment. In such a case, a report from your treating doctor or information from a previous flight doctor may be required in certain circumstances.

What class of Medical Certificate do I need?

  • First Class - Airline Transport Pilot
  • Second Class - Commercial Pilot, Flight Engeneer, Flight Navigator, Air Traffic Control Tower Operator
  • Third Class - Private Pilot, Recreational Pilot, Student Pilot

What is the validity of the Medical Certificate?

  • First - Class is valid for the remainder of the month of issue.
    • plus 6 - calendar months for operations requiring a first - class medical certificate if the airman is age 40 or over on or before the date of the examination,
    • or plus 12 - calendar months for operations requiring a first - class medical certificate if the airman has not reached age 40 on or before the date of examination 12 - calendar months for operations requiring a second - class medical certificate,
    • or plus 24 - calendar months for operations requiring a third - class medical certificate,
    • or plus 60 - calendar months for operations requiring a third - class medical certificate if the airman has not reached age 40 on or before the date of examination.
  • Second - Class Medical Certificate: A second - class medical certificate is valid for the remainder of the month of issue;
    • plus 12 - calendar months for operations requiring a second - class medical certificate,
    • or plus 24 - calendar months for operations requiring a third - class medical certificate,
    • or plus 60 - calendar months for operations requiring a third - class medical certificate if the airman has not reached age 40 on or before the date of examination.
  • Third - Class medical certificate is valid for the remainder of the month of issue.
